What do I do with a check that I received in the mail for the Lab work I had done with Everest?

Few insurance companies will mail the check to the patient instead of the provider. In that case, you can sign the back of the check and mail it to us with the explanation of benefits(EOB) at our address below. You can also cash the check and write a personal check to us if that is what you are comfortable with. Mail payments to:
